Home Care RN – Nights (On-Call)
Location: Monmouth County
The VNA Health Group is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. This vital role provides after-hours care and support to patients and families, ensuring exceptional clinical service and comfort in the home setting.
Responsibilities:
Combination of indoor and outdoor work; includes patient contact, use of clinical equipment, and electronic documentation.
Compensation:
In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is $60,000 – $110,000, which reflects base pay, overtime, bonuses, and other incentives. Actual compensation will depend on experience, skills, and internal equity.

Home Care RN – Nights (On-Call)

Responsibilities:
- Patient Care: Deliver high-quality nursing care during after-hours, addressing physical, emotional, and educational needs.
- Emergency Response: Respond promptly to calls, conduct home visits as needed, and manage urgent patient needs.
- Symptom Management: Assess and manage pain and other symptoms to promote patient comfort.
- Family Support: Provide guidance and reassurance to families during challenging situations.
- Collaboration: Communicate effectively with physicians, case managers, and interdisciplinary team members.
- Documentation: Maintain accurate, timely records of all patient care and communications.
- Adhere to Best Practices: Uphold VNA Health Group’s standards for patient-centered care and quality outcomes.
- Education: Current NJ RN license required.
- Experience: 1–2 years of home care, hospice, or acute care experience preferred.
- Skills: Excellent clinical assessment, critical thinking, and communication skills.
- Availability: Ability to work night shifts, including weekends and holidays as scheduled.
- Transportation: Reliable vehicle, valid driver’s license, and proof of auto insurance.
